  • Farm pay in cash needs a notarized employer letter on letterhead
  • W-2s and tax return for the current year
  • 12-month printout of child support and the court case order
  • Social Security and Supplemental Security Income award letters (current and past year)
  • Unemployment letter (wage transcript)
  • Children over 18: college transcript showing credits (12 per semester)
  • A notarized letter of no employment
  • If you have assets: a signed bank verification
  • If you pay day care for a child 13 or younger: day care verification form
  • Picked up before your appointment
  • Landlord receipt or notarized statement if you stay with family
  • Background check for every adult in the household
