- Birth certificates for all family members
- Social Security cards for all family members
- Income proof for all family members
- With 12 months of wages (also Social Security, pension, child support, food benefits)
- Last 6 months of checking statements and the last savings statement (include apps like Venmo or PayPal)
- Driver's license or photo ID for everyone 18 and older
- Custody papers and papers showing the value of any property you own
- Elderly or disabled only: Medicare supplement premium receipts and 12 months of pharmacy costs
