Do NOT mail. Call (217) 532-3672 ext. 221 to schedule an intake interview at 216 Shelbyville Road, Hillsboro, IL; every adult (over 18) must be present; office open Monday to Friday, 8 a.m. To 4 p.m., but closed to the public on Thursdays
Documents you'll need
- Completed application (all sections must be completed or it will not be accepted)
- Certified birth certificates for each member who will reside in the household
- Social Security cards for each member who will reside in the household
- Photo ID for all household members over 18
- Assets: names and addresses of banks or financial institutions with checking
- Savings
- CDs
- Stocks
- Bonds
- IRAs and other investments
- Proof of value for all real estate: appraisal and proof of any money owed
- Child care provider name and address (counted only if paid by you and not reimbursed
- Public Housing and Section 8 only)
- Landlord names and addresses for the last 3 years and addresses where you lived in that period
- Documentation of name changes: marriage certificates
- Divorce decrees
- Child custody documentation
- Disabled or elderly: names and addresses of medical providers for out-of-pocket expenses
- Medical insurance payment verification
- Pharmacy name and address for prescriptions
