- Background check form for every family member age 17 or older
- Mailed with the pre-application
- The authority lists papers it may ask for once your name is reached on a list. Do not send them with the pre-application
- Birth certificates for all household members (asked for when your name is reached)
- Social Security cards for all household members
- Current driver's license or ID card
- Pay stubs for the last six months for every adult
- Plus the latest tax return and W-2 forms
