In person

Get an application at 100 S Richard Pryor Place, or call (309) 676-8736 ext. 14079 to have one mailed to you. Return it in person only (Tuesday and Thursday, 8:30 am-12 pm and 1-3 pm). It cannot be mailed in or dropped off. All adults in the household must be there

  • Social Security cards for all household members
  • Photo ID for all household members age 18 and older
  • Five most recent consecutive paycheck stubs
  • Proof of income for all household members
  • Such as child support
  • Public aid or unemployment benefit letters
