• Mailed with the pre-application
• The authority lists papers it may ask for once your name is reached on a list. Do not send them with the pre-application
• Birth certificates for all household members (asked for when your name is reached)
• Social Security cards for all household members
• Current driver's license or ID card
• Pay stubs for the last six months for every adult
• Plus the latest tax return and W-2 forms
• Current letters for Social Security
• Pension
• Unemployment or cash assistance benefits
• The three most recent checking and savings statements for all household members
• Including children
• Copy of your current lease
• Or papers showing where you live
• Other papers they may ask for: divorce papers
• Immigration card
• Child support records
• Child care bill
• Student schedule
• Homeless preference: the McHenry County Continuum of Care verification form
• Turned in with the pre-application
