Documents you'll need

  • One month of recent income papers for each employed adult: four weekly pay stubs
  • Or an employer letter on letterhead
  • Most recent federal tax return with W-2s and 1099s for each household member aged 18 or older (two years for the self-employed)
  • Three months of the most recent statements for all bank and investment accounts
  • Latest quarterly statement for retirement accounts
  • A Statement of No Income form for any adult with no income who is not a full-time student
